29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about December 7, 2018: An employee was providing a piece of lumber to a worker on the roof which had tools such as but not limited t a circular saw and a pneumatic nail gun and was not using a hard hat; exposing the employee to head injuries.

29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects.  a) On or about December 7, 2018: An employee was operating a pneumatic nail gun and a circular saw while performing framing activities and was not using safety glasses to protect him from flying objects; exposing the employee to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about December 7, 2018: Four employees were observed installing roof sheathing on a roof with a pitch of 5:12 and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection; exposing the employees to a 10-feet fall hazard.   b) On or about December 7, 2018: Two employees were observed installing bracing elements and trim at the porch area with a pitch of 5:12 and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection; exposing the employees to a 13-feet fall hazard.
