29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative.    a. Anchor Staffing, Inc. worksite located at 2130 South Woodlane Circle, Bowling Green, Ohio:  On or about December 5, 2018 the employer did not ensure a lockout program was developed, documented, and utilized prior to employees performing unjamming activities on the clutch guide machine loading conveyor located adjacent to the heat treat department.    b. Anchor Staffing, Inc. worksite located at 2130 South Woodlane Circle, Bowling Green, Ohio:  On or about December 11, 2018 the employer did not ensure a lockout program was developed, documented, and utilized prior to employees performing unjamming activities on the clutch guide machine blast cabinet exit chute located adjacent to the heat treat department.    c. Anchor Staffing, Inc. worksite located at 2130 South Woodlane Circle, Bowling Green, Ohio:  On or about December 11, 2018 the employer did not ensure a lockout program was developed, documented, and utilized prior to employees performing unjamming activities on the clutch guide machine blast cabinet exit and entrance sensors.  Employees must use a hand tool to activate the sensors that open and close the entrance and exit doors and are exposed to crushing hazards from the door when activating the sensors.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     a. Anchor Staffing, Inc. worksite located at 2130 South Woodlane Circle, Bowling Green, Ohio:  On or about December 11, 2018 the employer did not ensure employees were protected from crushing hazards created by the vertical opening and closing of the end doors on the #2 clutch guide blasting machine located adjacent to the heat treat department.    b. Anchor Staffing, Inc. worksite located at 2130 South Woodlane Circle, Bowling Green, Ohio:  On or about December 5, 2018 the employer did not ensure employees were protected from shear point hazards created by the parts pusher on the end of the clutch guide machine #1 loading conveyor located adjacent to the heat treat department.
