29 CFR 1910.178(l)(4)(ii): The employer did not provide refreasher training to all employees engaged in the unsafe operation and use of forklifts:  (a) Nite Ize, Inc. at 6303 Dry Creek Parkway Longmont, CO 80503:  On or before December 10, 2018 the employer did not provide forklift refresher training to a forklift operator who was involved in an accident. A certified forklift operator struck a racking system with a forklift, reported the incident to management, and was not given any forklift refresher training.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical.  (a) Nite Ize, Inc. at 6303 Dry Creek Parkway Longmont, CO 80503:  On or before December 10, 2018 the employer did not maintain a safety data sheet for each hazardous chemical stored in the warehouse. These chemicals include but are not limited to organic silicon gross rubber, Li-Ion Polymer Battery, Colorant used on Silicon, and Alkaline Manganese Button Cell. This exposes employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  (a) Nite Ize, Inc. at 6303 Dry Creek Parkway Longmont, CO 80503:  On or before December 10, 2018 the employer did not ensure that portable fire extinguishers were inspected at least monthly in that the extinguishers in the facility had not been inspected since August of 2018.  This condition exposed the employees to fire hazards.
