29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    A) At the 11622 Lake Lucaya Drive where employees using pneumatic air guns and power circular saws while installing plywood sheets on the roof of residential construction were not provided with safety glasses. The employees were exposed to wood and nail ricochets eye hazards as observed on or about 12/11/2018.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    A) At the 11622 Lake Lucaya Drive where employees used an extension ladder that did not extend at least 3 feet (.9 m) above the eave of the roof. The employees were exposed to a fall hazard of approximately 9 feet 4 inches as observed on or about 12/11/2018.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     A) At the 11622 Lake Lucaya Drive where four employees installing plywood sheets on a residential roof were exposed to a fall hazard of 18 feet in that they were not using a fall protection system. As observed on or about 12/11/2018.      The J.G.R. Construction Group Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1196468, citation number 01, item number 01, issued on December 15, 2016 and affirmed as a final order on December 27, 2016, with respect to a workplace located at 628 Chatam Walk Dr., Ruskin, FL 33570.
