29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system. Location - Lupine Road, Andover, MA. On or about December 12, 2018, an employee was exposed to cave in while working/walking in and open excavation where no protection was provided for cave-in. J. W. Watson, JR. Excavation, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.652(a), which was contained in OSHA inspection number 1317275, citation number 1, item number 1, and was affirmed as a final order on July 9, 2018, with respect to a workplace located at 31 Lowell Street, Andover, MA.
