29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator:  a)  At the facility, on or about December 13, 2018 - An employee was not provided with a medical evaluation prior to allowing employees to voluntarily wear a half mask respirator.  An employee voluntarily wears a Miller LPR 100 half mask respirator during powder coating operations.

29 CFR 1910.134(e)(6): 29 CFR 1910.134(e)(6):  In determining the employee's ability to use a respirator, the employer shall obtain a written recommendation regarding the employee's ability to use the respirator from the PLHCP (physician or other licensed health care professional):  a)  At the facility, on or about December 13, 2018 -  A written recommendation regarding the employee's ability to use the respirator was not obtained from the PLHCP.  An employee voluntarily wears a Miller LPR 100 half mask respirator during powder coating operations.
