29 CFR 1910.1200(e)(1)(i): The employer did not compile a complete list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet. a) 20/20 Custom Molded Plastics located at 14620 Selwyn Drive in Holiday City, Ohio 43543; On or about December 13, 2018 the employer did not include combustible dust in the list of hazardous chemicals known to be present in the work place as part of the companys hazard communication program. Employees working at or near the injection molding presses were exposed to combustible dust hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) 20/20 Custom Molded Plastics located at 14620 Selwyn Drive in Holiday City, Ohio 43543; On or about December 13, 2018 the employer did not ensure employees were trained on the hazards in their work area. Employees were exposed to explosion and deflagration hazards in that the plastic fines conveyed in the pneumatic dust collection system was combustible.
