Location: c/o Broad St. & Clark St., Newark, NJ. a) Employees involved in masonry operations at the job site were working from a tubular welded scaffold that was not fully planked at the working level. The employee was exposed to a potential fall hazard of up to approximately 6 ft. to the lower level; on or about 12/13/18.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Location: c/o Broad St. & Clark St., Newark, NJ. a) Safe means of access was not provided as masonry workers gained access to the welded tubular frame scaffold by climbing the side rails, thereby exposing workers to fall hazards; on or about 12/13/18.
