Employees used a JBC forklift, Serial Number 10851089 to lift materials and lower trash from the roof.

On or about December 13, 2018 and at times prior to, employees working on the roof were exposed to fall and struck-by hazards. The employer did not provide training to the employees operating an All-Terrain Vehicle (ATV) with a trailer hitch on a 20,000 square foot flat roof transporting material. The owner's operator manual was not provided for employees use and employees were not familiar with the hazards and means to control the hazards associated with using the ATVs.

The ground pin was broken off from the attachment plug, exposing employees to injuries from electrical shock.
