29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about December 13, 2018, during the installation of siding material at a job site located at 9703 Baltic Rd. Cleveland, OH 44102 the employer did not ensure employees working on a residential roof were protected from a fall of a height of approximately 11 feet on a 2:12 pitch roof by utilizing fall protection systems/devices.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about December 13, 2018, while installing siding materials off a ladder jack scaffold at a residential home located at 9703 Baltic Rd. Cleveland, OH 44102, the employer failed to protect employees from an approximately 17.2 foot fall by using fall protection. JWW Home Improvement Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1126854, citation number 1, item number 1 and was affirmed as a final order on 3/22/2016, with respect to a workplace located at 18746 Ledge Stone, Strongsville, OH 44136.
