29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent. MJ Holding Company, LLC, Bedford Park, Illinois - On or about December 13, 2018, the employer forms used for recording injuries and illnesses were not an equivalent form for the calendar year 2018. The form used in place of the OSHA 300 did not include the case number (column A), whether the case involved a death (column G), whether the case involved days away from work (column H), whether the case involved a job transfer or restriction (column I), whether the case involved an other recordable case (column J), the number of days away from work (column K), the number of days of on the job transfer or restriction (column L), and whether the case involved and injury or illness (column M1-M6). a) On or about September 10, 2018, a recordable injury occurred when a general labor employee sustained an injury to the left knee and head after tripping over a pallet, resulting in medical treatment beyond first aid. b) On or about September 12, 2018, a recordable injury occurred when a general labor employee received a lower back strain when lifting a 15 pound box, resulting in at least four (4) days of restricted duty. c) On or about November 14, 2018, a recordable injury occurred when a general labor employee sustained a left knee sprain while attempting to step from one skid to another. The employee missed the skid and stepped to the ground instead, resulting in one (1) lost day of work.
