Super Torque gantry roller presses or the one (1) Production Equipment and Engineering, Co. gantry roller press to protect the employees from the roller press point of operation, ingoing nip points at the gantry wheels and rails, and the transverse movement of the roller press carriage.    (b) On or about 12/12/2018 - Diamond Line, guarding was not provided on the two (2) Mitek Industries, Inc. gantry roller presses to protect the employees from the roller press point of operation, ingoing nip points at the gantry wheels and rails, and the transverse movement of the roller press carriage.

(b) On or about 12/12/2018 - Between the Jack Table and Virtek Line, a guard was missing on the utility clamp work light with an incandescent bulb and the bulb protruded beyond the work light's reflector.    (c) On or about 12/12/2018 - Cyber Saw 2 Area, a guard was missing on the utility clamp work light with an incandescent bulb.

(b) On or about 12/12/2018 - Diesel Fuel Storage and Dispensing Area, a warning sign stating "No Smoking" was not posted.  (c) On or about 12/12/2018 - Virtek Line at Gantry Roller Press 3, the Danger and Caution signs were covered up approximately 75% with documentation that was taped to the machine.
