29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    Amaral Revite Corporation, Burger King, 344 King St., Northampton, Massachusetts; On and about December 13, 2018, subcontractor employees were in a trench over 100 feet long with no ladders available for egress.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:    Amaral Revite Corporation, Burger King, 344 King St., Northampton, Massachusetts; On or about December 13, 2018, areas of up to 1 foot of the asphalt on the road had been exposed and were unsupported, exposing subcontracted employees to struck by hazards from falling material.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.    Amaral Revite Corporation, Burger King, 344 King St., Northampton, Massachusetts; On and about December 13, 2018 subcontracted employees were exposed to materials falling or rolling into the trench.    a.  The excavator staddled the trench and was right on the edge at the end away from the street.  b.  The spoil piles on the north side of the trench were less than 2 feet from the edge of the trench.
