Richard Seger Construction, on or about December 13, 2018 at the address of 4142 Douglas Rd. Toledo, OH: The employer did not assure that employees operating the nail gun during roofing work wore eye protection to protect from flying object hazards.

Employees were exposed to approximately 20 ft. falls to the ground below.
