29 CFR 1915.89(b): A written program and procedures for lockout and tags-plus systems to control hazardous energy that included the provisions in 29 CFR 1915.89(b)(1) - (b)(6) was not established and implemented while servicing machinery, equipment or systems: a) Naval Station Norfolk, Norfolk, VA 23511, U.S.S. Mason 5-8-0-T: On or about December 18, 2018, and times prior thereto, the employer did not have a written program for lockout/tags-plus for employees that performed work on the ship's degaussing system with 120VAC and 450VAC electrical energy sources, exposing employees to electrical hazards. Navy ship's force maintained control of the machinery, equipment or systems on the vessel. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.12(d)(5): The employer did not certify that the training required by paragraphs (d)(1) through (d)(4) of this section had been accomplished: a) Naval Station Norfolk, Norfolk, VA 23511, U.S.S. Mason 5-8-0-T: On or about December 18, 2018, and times prior thereto, the employer did not certify that each employee received required training prior to confined space entry. Employees performed electrical systems work in a ship's void/confined space that required a Marine Chemist certification prior to entry and hot work, exposing employees to potential atmospheric and other hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
