29 CFR 1926.451(b)(2)(ii): When walkways between scaffold sections are less than 18 inches wide, employees were not protected from falls by guardrail systems or personal fall arrest systems:  (a) Around the exterior of house - On or about December 13, 2018 employees were exposed to a fall hazard.  The employees framing a new house were working from a wooden scaffolding the had the walkway planks 5 1/2 inches wide with no guard rails or fall arrest system.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.Note:  See exceptions in paragraphs (b)(3)(i) and (ii) of this section.  (a) Around exterior of house walls - On or about December 13, 2018 employees were exposed to a fall hazard.  The employees were walking on the scaffold walkway plank framing a new house that was 23 inches away from the house wall without any guard rails or fall arrest system.

The employees framing a new single story house were working from a job made wooden scaffolding that has the walkway plank spliced and is not resting on a supported surface.

The employees framing a new single story house were using a wooden scaffolding that had no mud sill plates under the supports and the scaffolding was sinking into the ground.

(a) Top of roof - On or about December 13, 2018 employees were exposed to a fall hazard. The employees applying roof plywood sheeting on steep roof 9 feet 6 inches high were not protected from falls with the use of a fall protection system.
