29 CFR 1910.26(a): Dockboards were not capable of supporting the maximum intended load in accordance with �1910.22: Worksite located at Papas Dodge, Inc., 585 East Main Street, New Britain, Connecticut: On, or about, December 10, 2018, the Vestil Model AH-3630 dock plate provided for use by delivery drivers was not designed to support the maximum intended load in that the weight limit for the dockplate was 700 pounds. For a diesel engine block, weight of driver guiding car and cart weight, the load could exceed 700 pounds.

29 CFR 1910.26(c): Portable dockboards were not secured by anchoring them in place nor by using equipment or devices that prevented the dockboard from moving out of a safe position: Worksite located at Papas Dodge, Inc., 585 East Main Street, New Britain, Connecticut: On, or about, December 10, 2018, the Vestil Model AH-3630 dock plate provided for use by delivery drivers was not long enough for the height differential between the trailer dock and dock surface. As a result, there was insufficient contact on the both the trailer surface and dock surface to prevent the dock plate from moving as the cart was moved. Also, to minimize the angle of the dock plate, drivers would leave a gap of several inches between the trailer and dock, which also allowed the dock plate to move.

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form. Located at the Aero Global Logistics LLC, 11 Suffolk Road, Mansfield, Massachusetts: On or about December 17, 2018, the employer failed during calendar year 2017, to electronically submit information from their OSHA Form 300A or equivalent by July 1, 2018. a). The establishment employed 54 employees and is classified 484110 during calendar year 2017.
