29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) On or about December 11, 2018, workers were exposed to fall hazards while using personal fall arrest systems and not adjusting the rope grabs up to a position on the rope (vertical life line) as close to the anchor as possible to permit work at the necessary location; rather, the ropes had long slack in them which, if the workers fell, would permit the workers to contact objects below or even the ground. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about December 11, 2018, workers who were exposed to fall hazards were instructed to use Personal Fall Arrest Systems, but were not trained in the proper way to use the systems. An improperly engaged system can result in serious injury if a user falls, either from the system failing to stop the fall, or from the system causing harm to the wearers. The workers did not tighten the straps of the harnesses and some did not buckle all of the harness buckles. Abatement Note: The employer shall verify training by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer (29 CFR 1926.503(b)(1)). Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
