29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems: (a)Davis & Sons Millwright and Repair Service LLC, at 207 US-18, Davis, SD 57021: On or about and at times prior to December 14, 2018, the employer did not ensure that employees were protected from falls while installing roofing material on a commercial property. Employees were working on the roof and on open trusses with no fall protection systems in place. This condition exposed employees to a fall hazards of 14 feet and 17 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a)Davis & Sons Millwright and Repair Service LLC, at 207 US-18, Davis, SD 57021: On or about and at times prior to December 14, 2018, the employer did not ensure that each employee engaged in roofing operations on a commercial building, had been trained to recognize the hazards associated with working at elevations over 6 feet. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet. The employer did not provide training on the simple saver fall protection system involving it's limitations and use. The condition exposed employees to the hazard of falling from an elevation of approximately 14 and 17 feet.

Employees were using an extension ladder to access the roof to install roofing material. This condition exposed employees to a fall hazard of approximately 14 feet.
