29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:      (a)  On or about December 11, 2018, employees were exposed to fall hazards of 11 and greater while roofing a house without  fall protection. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about December 11, 2018, workers who were exposed to fall hazards were instructed to use Personal Fall Arrest Systems, but were not trained in the proper way to use the systems.  An improperly engaged system can result in serious injury if a user falls, either from the system failing to stop the fall, or from the system causing harm to the users.  The workers did not properly tighten the straps of the harnesses, and some did not buckle all of the buckles of the harnesses.  (b)  On or about December 11, 2018, workers were exposed to injury from falls while wearing personal fall arrest systems that were incorrectly connected.  A rope (vertical life lines) with a shock absorbing lanyard was wrapped around a roof truss and the double acting locking connector was attached to the rope rather than a dedicated anchor. Were the wearer to fall, the shock absorber would not be able to deploy thus transmitting the full force of the fall to the wearer's body which could result in serious injury.    Abatement Note:  The employer shall verify training by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer (29 CFR 1926.503(b)(1)).  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) On or about December 11, 2018, employees were exposed to fall hazards from the ladders used to access the roof, which did not extend at least 3 feet above the landing surface.
