29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about December 13, 2018 at 1200 Opelika Rd., Opelika, Al., Building 319: An employee was installing sheathing at/near the roof edge  on a new residential unit without using fall protection which exposed him to fall hazards of 24 feet 5 inches to the ground below.  b) On or about December 13, 2018 at 1200 Opelika Rd., Opelika, Al., Building 319: An employee was cutting sheathing and passing it up to the employee on the roof installing it from a balcony which was not equipped with mid-rails, exposing him to fall hazards of 11 feet, 5 inches to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about December 13, 2018 at 1200 Opelika Rd., Opelika, Al: An employee used an extension ladder to gain access onto the roof of a new residential unit in which the side rails did not extend at least three feet above the landing area which exposed the employee to fall hazards of 24 feet 5 inches when getting on/off the ladder.
