29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. This was most recently observed on December 13, 2018 at the corner of 208th St and Boyd St, Elkhorn, Nebraska, 68022. Four employees were working on the roof of a residential structure without utilizing fall protection. All employees were exposed to falls of over 21 feet to the dirt ground below. The pitch of the roof was greater than 4:12. Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1127759, citation number 1, item number 1, and was affirmed as a final order on April 3, 2017 with respect to a workplace located at 6611 S. 200th Ave, Omaha, NE 68135.
