(a) 910 Old Vicksburg Rd - On or about November 20, 2018 the employer had not adequately performed training on fall hazards, for employees that were required to wear personal fall arrest systems, prior the beginning work on the water tower while performing maintenance and repairs exposing employees to the hazards of falling.

29 CFR 1926.1060(a)(1):  The employer did not ensure that each employee was trained by a competent person which would enable each employee to recognize hazards related to ladders and stairs, and the procedures to follow to minimize those hazards.    (a) 910 Old Vicksburg Rd, Clinton MS - On or about December 7, 2018 the employer did not ensure employees had received adequate training on the safe use of ladders at the workplace. Employees were exposed to fall hazards in excess of 55 feet from the top of the tower to the middle section due to not receiving adequate training on procedures and requirement of attaching lanyards of personal fall arrest system to the ladder safety devices.

This training must result in an understanding of the hazards in the permit space and the methods used to isolate, control or in other ways protect employees from these hazards, and for those employees not authorized to perform entry rescues, in the dangers of attempting such rescue:    (a) 910 Old Vicksburg Rd, Clinton MS - On or about December 10, 2018 The employer did not ensure employees working in confined spaces had received adequate training exposing employees to asphyxiation hazard while performing sandblasting, painting, maintenance and repairs inside of a water tower.
