29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   A) Front of house, on or about December 17, 2018, and / or at times prior to, an employee was working from the roof of a residence without any type of fall protection equipment or other method allowed by the standard, exposing the employee to a 10-foot fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A) Front of house, on or about December 17, 2018, and / or at times prior to, an employee was using a metal extension ladder to access the roof of a house. The ladder did not extend at least three feet above the eve of the roof, exposing the employee to a 10-foot fall hazard.
