29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On the east side of the construction site, employees engaged in framing activities were not wearing hardhats while working below workers. The employees were exposed to struck-by hazards from falling tools and material.

29 CFR 1926.304(d): When the tool was withdrawn from the work, the lower guard did not automatically and instantly return to the covering position. On the east side of the construction site, employees operating a Milwaukee skill saw with the lower guard wedged in the up position while cutting framing material were exposed to amputation hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On the east side of the construction site, employees who worked without fall protection were exposed to fall hazards of approximately 11 feet while engage in framing activities.
