29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about December 17, 2018, one subcontracted employee was not protected by a hard hat while working below roof trusses that were being lifted by a Gradall Forklift, exposing employees to the hazard of being stuck-by falling building materials.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about December 17, 2018, two subcontracted employees were standing and sitting on the top of the wall installing roof trusses on the commercial property under construction, exposing the employee to a fall hazard over 6 feet.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.  On or about December 17, 2018, subcontracted employees were using the wood frame of the commercial property under construction to gain access to the second floor, exposing employees to a fall hazard of approximately 27 feet.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed:  On or about December 17, 2018, one employee was using a 8 foot Louisville A-Frame Ladder that was leaned against the side of the commercial property under construction, the ladder was not set up and used for the purpose which it was designed, exposing employees to a fall hazard of approximately 6 feet.
