29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed. On or about November 28, 2018, during the removal/installation of foundation material along with trenching/excavation operations at 1210 Crankford Ave, Lakewood, Ohio 44107 the employer did not conduct frequent and regular inspections of the job site where employees were exposed to cave in hazards where employees were working in a trench with varying depths of 6 to 7 feet, employees were utilizing a self-supporting ladders collapsed on to itself and leaned against the trench wall for access and egress.
