29 CFR 1910.178(l)(4)(ii)(E): Refresher training was not provided to forklift operators after a condition changed that affected safe operation of the truck: On December 10, 2018, an employee operating a Hyster 35 (SN#F001V02692E) sustained an amputation injury as a result of the forklift getting stuck in soft ground and fresh gravel. The employer had not provided refresher training to employees after learning of forklifts getting stuck on previous dates. The work conditions changed which affected the safe operation of the truck.
