29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    Location - 8 Nelson St. Lawrence, MA    a) Working on the roofs edge.    b) Working from a ladder installing insulation panels.    On or about December 10, 2018, employees were exposed to electrocution hazards while work from a ladder and on the roofs edge.    The Maxwell Trejo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.416 (a)(1), which was contained in OSHA inspection number 992895, citation number 1, item number 1 and was affirmed as a final order on October 19, 2015, with respect to a workplace located at 91-95 Newbury St. Lawrence, MA.

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled.    Location - 8 Nelson St. Lawrence, MA    On or about December 10, 2018, employees were exposed to falls from a improperly secured aluminum pump jack scaffold.    The Maxwell Trejo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.452 (j)(2), which was contained in OSHA inspection number 992895, citation number 1, item number 4 and was affirmed as a final order on October 19, 2015, with respect to a workplace located at 91-95 Newbury St. Lawrence, MA.
