29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On or about 12/11/18 at a roof replacement project located at 31 Saybrook Drive, Lancaster, New York; employees were installing shingles on the roof of a new house. The employees were accessing the roof with an extension ladder and the side rails of the ladder did not extend 3 feet above the landing surface.    NO ABATEMENT CERTIFICATION

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 12/11/18 at a roof replacement project located at 31 Saybrook Drive, Lancaster, New York; employees working on the roof were placing shingles on the roof of a new house. The roof had a 6 in 12 pitch and eave heights of approximately 10 feet. There was no fall protection in use.    NO ABATEMENT CERTIFICATION REQUIRED    D J Weaver Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1234465, citation number 1, item number 1 and was affirmed as a final order on 9/29/17, with respect to a workplace located at 34 Sterling Place, Lancaster, New York.
