29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Employee(s) engaged in steel erection work at a commercial building under construction at 16301 W. 87th Street, Lenexa, Kansas were exposed to hazards such as but not limited to falls, struck-by, crushing, and other construction hazards in that employee(s) were not instructed in the recognition of construction hazards and the methods to mitigate those hazards.

29 CFR 1926.501(b)(2)(i): 29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:     Employees engaged in leading edge work at a commercial building under construction located at 16301 W. 87th Street, Lenexa, Kansas were exposed to a fall hazard of approximately 18 feet due to no fall protection system being used at the job site.
