29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On the west side of the construction site, employees engaged in decking activities used pneumatic nail guns without wearing eye and face protection and exposed to struck-by hazards and eye injuries.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On the west side of the construction site, employees without fall protection were exposed to fall hazards of approximately 12 feet while engaged in decking activities.
