29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. 1: On or about December 19, 2018, the employer did not ensure that each affected employee used appropriate eye protection while using woodworking equipment, such as the Powermatic table saw (serial number unknown). Employees were exposed to eye injuries from flying wood chips.

29 CFR 1910.213(c)(1): Circular hand-fed ripsaw(s) were not guarded by an automatically adjusting hood, which completely enclosed that portion of the saw above the table and above the material being cut. On or about December 18, 2018, inside the shop on 11417 cloverland Ave, the employer allowed employees to operate a Powermatic table saw (serial number unknown) which was not guarded at the blade, exposing employees to point of operation hazards.
