Employee was permitted to voluntarily wear a tight-fitting respirator while conducting plasma cutting.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    4 Carleton Drive, Georgetown, MA    On or about December 19, 2018, medical evaluations were not completed for employees that utilized a tight-fitting half face respirator. An employee that was permitted to voluntarily wear a the tight-fitting half-face respirator while completing daily job tasks such as plasma cutting aluminum sheet metal was not provided a medical evaluation to determine the employee's ability to wear the respirator.

Employees were exposed to point of operation hazards that were not guarded to prevent employees from entering the danger zone, including but not limited to the following instances;    a) On or about October 24, 2018, an employee operating a metal shear machine in the main shop was exposed to a point of operation hazard due to having their hands in the danger zone during the operating cycle. Sections of the guards were easily moved up, and an employee suffered an amputation of two fingertips.

Main shop floor.    a) On or about December 19, 2018, an employee was welding a metal component in the open warehouse area without positioning a noncombustible or flameproof screen or shield to protect adjacent persons from the rays.    b) On or about December 19, 2018, plasma cutting being conducted on a plasma cutting machine was not shielded by a noncombustible or flameproof screen or shield to protect employees in the area of the machine.

On or about October 24, 2018, a temporary worker operating a metal shear machine in the main shop suffered an amputation injury to his right ring and middle fingers and the injury was not reported to OSHA within 24 hours.
