29 CFR 1926.552(a)(1): The employer did not comply with manufacturer's specifications and limitations applicable to the operation of all hoists and elevators: A. On or about December 19, 2019, 100 Old Andover Road, Hebron, CT jobsite: Two employees were allowed to climb the rungs of the TranzSporter Platform Hoist TP250. The splice plates holding the two twelve-foot track sections were not secured with the required number of carriage bolts and keeper nuts. The cable was not smooth. The weight of the two employees exceeded the 230 lb. load capacity of the equipment. The splice plate on the right side of the track released under the weight of the employees, throwing them approximately twelve feet to the ground below.
