29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person. On December 19, 2018, at 745 E. 4th St. Chillicothe, a two-story home, a ladder jack scaffold was erected by an untrained employee not under the supervision and direction of a competent person in that the scaffold was erected without a fall protection system when the employee was working over 10 ft. off the ground.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; On December 19, 2018, at 745 E. 4th St., Chillicothe, a two story home, an employee was exposed to a 17 ft. fall hazard while performing soffit work on a ladder jack scaffold without utilizing any fall arrest system.

29 CFR 1926.454(a)(1): The nature of any electrical hazards, fall hazards and falling object hazards in the work area; and requirements for the utilization of fall protection. The employer had not trained an employee on the nature of fall hazards and the utilization of fall protection while working on a ladder jack scaffold approximately 17 ft. off the ground without utilizing fall protection to perform soffit work on the side of the two story home at 745 E. 4th St. Chillicothe on December 19, 2018.
