On or about December 19, 2018, the employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed and the dates of the hazard assessments. The employer had written PPE assessments for employees working on Line 105 and 106 that addressed hazards which could affect employee's Eyes, Head/Neck/Face, Respiratory, Hearing, Hands/Arms, Torso, and Feet, but did not have the assessments dated.

29 CFR 1910.134(e)(6)(i): Obtain a written recommendation regarding the employee's ability to use the respirator from the PLHCP. The recommendation shall provide only the following information: a. On or about December 19, 2018, the employer did not obtain a written recommendation from the PLHCP regarding an employee's ability to use a respirator while working on Line 105. The employee uses a 3M belt-mounted powered air-purifying respirator model number GVP-100 multiple times for a total of approximately one hour a day, ten days per month when adding maleic anhydride as part of a production process.
