29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors from sources such as open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat:    On or about December 14, 2018, an employee was exposed to fire hazards that resulted in second degree burns to the hand, back, and other body parts. The employee was performing a washing process that required transferring toluene from an unbonded and ungrounded 55 gallon drum via a hand pump. The employer did not ensure adequate precautions were taken to prevent the ignition of flammable vapors from sources such as, but not limited to, static electricity.    There is no abatement certification or documentation required for this item.

Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.    On or about December 14, 2018, an employee was exposed to fire hazards that resulted in second degree burns to the back and other body parts. The employee was performing a washing process that required transferring toluene from an unbonded and ungrounded 55 gallon drum via a hand pump.
