29 CFR 1910.147(f)(1)(ii): Testing and positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the from the energy isolating device and the machine or equipment energized to test or position the machine, equipment, or component thereof, the following sequence of actions shall be followed: Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section: a) On December 12, 2018, the Line #1 Strut Machine Operator in the Pour and De-bridge Department was jogging extrusion material through the crimping disks to check the alignment. The Line #1 Strut Machine Operator's left hand was adjusting the crimper disc roller while jogging the extrusion material when his left index finger was pulled into the ingoing nip point created by the crimping disk and extrusion material.
