29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  (a) New residential construction located on Lot #3, Front side: On or about 11/29/2018, and at times prior, the employer exposed employees to struck-by hazards while operating an Hitachi pneumatic framing nail gun, in that no protective eyewear was provided to employees engaged in framing activities while operating these tools at 120 PSI.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a) New residential construction located on Lot #3, Front side: On or about 11/29/2018, and at times prior, the employer exposed employees to fall hazards of approximately 21-feet, 4-inches to the lower level, in that a fall protection system was not provided to an employee working from a job-made window jack scaffold.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) New residential construction located on Lot #3, First floor: On or about 11/29/2018, and at times prior, the employer exposed employees to fall hazards of approximately 12-feet to the lower level, in that the ladder provided to access the upper level was not extended at least 3-feet past the landing or secured to the structure to allow for safe access to all levels.
