OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing type hazards while holding a wood block in place in close proximity to forklift operations: (a) Worksite: On or about 12/20/2018, the employer did not ensure that the method for moving a Pelletizer machine with a forklift did not involve a pedestrian holding a wood block in place as the forklift tines are raised against the wood block and the frame of the machine being lifted.
