29 CFR 1926.50(c): A person who has a valid certificate in first-aid training was not available at the worksite to render first-aid: (a) Benchmark Roofing, LLC located at 43142 Needle Rock Road Crawford, CO 81415: On and preceding 20 December, 2018, the employer did not ensure that a person who had a valid certificate in first-aid training was available at the worksite to render first-aid. Four employees were conducting a re-roofing project on a remotely located residential home. Following an employee injury from a fall, it took 45-60 minutes for emergency responders to arrive. The delay in medical care exposes employees to injuries progressing in severity due to inadequate training to administer first aid.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) Benchmark Roofing, LLC located at 43142 Needle Rock Road Crawford, CO 81415: On and preceding 20 December, 2018, the employer did not ensure that a work-related incident resulting in in-patient hospitalization was reported within 24 hours. Workers were re-roofing a residential home when an employee fell approximately 28 feet. The fall resulted in a hospitalization 12/18/2018 at approximately 1:15 PM. The employer did not report the hospitalization.
