29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     On or about December 24, 2018, at the employers work site located at 8383 Bayard Rd, Minerva, Ohio 44657; the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections.  Employees were not protected from the unexpected energization, startup, or release of stored energy while performing servicing and maintenance activities on the INSTA-PRO OIL PRESS Model 5005, Serial # 177 (Soy Bean Oil Press).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. On or about December 24, 2018, at the employers work site located at 8383 Bayard Rd, Minerva, Ohio 44657; the employer did not ensure the INSTA-PRO OIL PRESS Model 5005, Serial # 177 machine operator(s) were protected from the rotating parts of the Insta-Pro Oil Press auger blades that force material dumped into the upper hopper into the machine.    b.  On or about December 24, 2018, at the employers work site located at 8383 Bayard Rd, Minerva, Ohio 44657; the employer did not ensure the INSTA-PRO OIL PRESS Model 5005, Serial # 178 machine operator(s) were protected from the rotating parts of the Insta-Pro Oil Press auger blades that force material dumped into the upper hopper into the machine.
