Location: a) At 143 Chambers Street, New York, NY 10007    On or about 12/06/2018, employees used a straight ladder for access between the cellar level and the first floor level on the northeast side of the jobsite. The ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed. The ladder also was not secured at its top.
