29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On December 19, 2018 , located inside the production area of the building at 40 Commercial Avenue, Middletown, NY, an employee was repairing a chain drive on the circular sorting table without un-plugging the unit. The employee accidently activated the motor for the chain resulting in a fingertip amputation.
