29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Northeast side of building roof - On or about December 27, 2018 and at times prior thereto, employees were framing the roof of the building using a rope grab system for fall protection. The anchor point was in the center of the building and employees were working on the East side of the building approximately 50' away from the anchor point, allowing them to contact the ground in the event of a fall.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Southwest side of building - On or about December 27, 2018 and at time prior thereto, an employee was framing the roof of the building, walking along the trusses, without the use of fall protection. The 518 Contracting, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1290733, citation number 1, item number 1 and was affirmed as a final order on March 5, 2018, with respect to a workplace located at 43 Jane Street, Saratoga Springs, NY 12866. The 518 Contracting, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1021981, citation number 1, item number 1 and was affirmed as a final order on March 20, 2015, with respect to a workplace located at 318 Ballston Ave., Saratoga Springs, NY 12866.
