At the job site on or about December 28, 2018, and at times prior thereto, the store facility was not maintained in a safe and sanitary condition, such as but not limited to, the ceiling tiles, rainwater and mold, exposing employees to possible struck-by, slip and falls and health hazards.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: At the main electrical panel located in the back storage room of the job site. The main electrical panel was blocked by stacks of boxes obstructing access to the electrical panel.
