29 CFR 1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment. (a) Gin Area - on or about December 19, 2018 the employer failed to develop instruction for employees which would include specific procedures for locking out equipment during maintenance and repair. The employee could suffer an injury of an amputation or broken limb to lacerations.

29 CFR 1928.57(d)(1)(iv): 29 CFR 1928.57(d)(1)(iv): Pulleys of V-belt drives were not completely enclosed or guarded by location whether or not railings are present. The open end of the pulley guard shall be not less than 4 inches from the periphery of the pulleys. (a) Gin Area - On or about December 19, 2018 employees were exposed to unguarded belt and pulley. Employees injury may be broken limbs or amputation from being caught in the belt.
