29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  1189 Pipeline Place - Employees in the process of decking the 6 in 12 slope (vertical to horizontal) roof, were exposed to an 20'8" fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 12/28/2018.  Esmel Construction Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1298785, citation number 1, item number 1 and was affirmed as a final order on  5/9/2018, with respect to a workplace located at 4505 Hudson Lane, Tampa, Fl.  Esmel Construction Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1299539, citation number 1, item number 1 and was affirmed as a final order on  6/18/2018, with respect to a workplace located at 3507 N. Tampa St., Tampa, Fl.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a)  1189 Pipeline Place - Employees were exposed to a 9'4" fall hazard, in that, the extension ladder did not extend 3 foot above the roof edge, observed on or about 12/28/2018.  Esmel Construction Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1298785, citation number 1, item number 2 and was affirmed as a final order on  5/9/2018, with respect to a workplace located at 4505 Hudson Lane, Tampa, Fl.
