29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board:    a. Site - An Employee who was using a "Chicken Ladder" to travel between the eave and the peak of the roof in order to transport bundles of shingles without the use of either fall protection or a grab line.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. Site - Employees using a  40 foot aluminum extension ladder to gain access to and from the roof were not protected from falls of approximately 32 feet when the side rails of the ladder did not extend at least 3 feet above the roof line.
